Scope of Services: hexadyn provides wellness services, including the prescription of medications within the limits of applicable laws and professional guidelines. Prescriptions are issued at the sole discretion of our licensed healthcare professionals based on clinical evaluation.
Health Insurance: hexadyn does not accept or process any form of health insurance. All services must be paid out-of-pocket at the time of service unless otherwise specified in writing.
Medical Disclaimer: While we provide prescription services, our offerings are not a substitute for comprehensive medical care. We encourage clients to maintain relationships with primary care providers for ongoing health management and to seek emergency medical care when necessary.
Patient privacy disclaimer: hexadyn prioritizes the privacy and security of our clients' personal information. While we do not accept insurance and are not classified as a covered entity under the Health Insurance Portability and Accountability Act (HIPAA), we are committed to maintaining high standards of confidentiality and data protection. We implement robust privacy practices to ensure that your personal information is handled with the utmost care and respect.
Payment Terms: Clients are responsible for full payment at the time of service. We accept venmo, credit card (with a 2.5% charge for processing) or cash if in person.
Changes to Terms: We reserve the right to modify these Terms of Service at any time. Continued use of our services after such changes constitutes acceptance of the updated terms.
